Special Needs Registration

Recent conversations in the community with residents alerted us to a much needed update to our system to be more inclusive.

The Collierville Fire Department and Collierville Police Department have introduced a Special Needs/Home Concern form to our website here: colliervilletn.gov/SpecialNeeds

This form is voluntary, free, and confidential and allows families and loved ones to provide information that may be vital in the event of an emergency. 

If you or someone you know can answer YES to any of the following questions, the Home Concern system could be for you:

  • Do you have a communication disorder or intellectual disability that affects your ability to communicate? (hearing impaired, language or speech disorders, use a communication device such as a “talker”)
  • Do you have a visual impairment?
  • Do you use life-supporting devices (oxygen, ventilator, feeding tube, etc) that require electricity?
    Have you had a stroke or a head injury?
  • Do you suffer from dementia or Alzheimer's disease?
  • Are you paralyzed in any way or bedridden?
  • Are you dependent on an assistive device (walker or wheelchair) to move around?
  • Do you suffer from a mental health diagnosis that we should be aware of (anxiety, depression, or schizophrenia)?
  • Would you need special transportation in the event of a disaster or evacuation (4WD, ambulance or wheelchair accessible vehicle)?
  • Do you use service animals to assist you in day-to-day activities?

Take the steps to prepare for a potential emergency by registering with the Collierville Police / Collierville Fire for the Special Needs/Home Concern Form.

We hope that this form can not only aid us in better serving our community, but also help provide some relief to family members and loved ones of our residents of all abilities.
